Coach and Volunteer Clearances
Pennsylvania state law and Conestoga Valley Youth Atheltic Association require all volunteers to have proper clearances. Persons interested in volunteering for a coaching position for any of our teams, CVYA board members and volunteers directly interfacing with players, are required to complete criminal background checks and receive state clearance in accordance with PA Law BEFORE they participate in ANY activity related to the CVYA programs. Pennsylvania Child Abuse History Clearance
No Cost. Valid for 5 years.

Complete all fields
Select: Volunteer
Volunteer Category: Other
Agency Name: Conestoga Valley Youth Softball
Print out the certificate. Write down or remember your login and password to access the certificate at any time.
PA State Police Criminal Record Check
No Cost. Valid for 5 years.

Click on New Record Check (Volunteers Only) and complete all fields
Print the "Record Check Results Page"
IMPORTANT: Click on "CERTIFICATION FORM" above the BACK button to print your actual certificate.
Volunteer Affidavit Form (if you have been a resident of Pennsylvania for 10 consecutive years)
OR
FBI Criminal History Clearance (if you have NOT been a resident of Pennsylvania for 10 consecutive years)

All required document PDFs must be turned into the CVYS Board before any practice is held. It is recommended that you save your clearances, as they can be used for other organizations.
